It will be easier to complete this form ON YOUR COMPUTER rather than your on phone.
Our 4-STEP APPROACH makes coaching a breeze…
For your convenience, our Coaching Intake Form will walk you through the above process in 7 simplified steps…
If you already recorded a presentation:
Save your presentation file under the following Video ID name format:
Your first and last name plus the last 4 digits of your phone number, plus today’s date (e.g. John Smith 3573 2020-06-23). Remember or write down your Video ID name. You will have to use it later in the intake process.
Then go directly to Step 2 below.
If you have not yet recorded your presentation:
You will have to record your presentation before completing this Coaching Intake Form.
Please note that the internet is never 100% secure. Therefore, we encourage you NOT to upload presentations that contain confidential or sensitive material/information.
In most cases, you will be recording a rehearsal for an upcoming presentation. For your convenience, you can click on the brown buttons below to access recording tips for various rehearsal scenarios…

SAVE ON A PHONE: If you recorded on a smartphone, your video will automatically be saved on your phone.

Specific recording tips are provided below…
Find a QUIET ENVIRONMENT where you won’t be disturbed. Ideally, try to record your rehearsal at the venue where you will be presenting.
Ensure you have SUFFICIENT LIGHTING.
Ideally, wear the SAME CLOTHES AND ACCESSORIES that you will wear for your actual presentation.
If you will be using a LECTERN at your actual presentation, try to use one during your rehearsal. You can use a raised table or music stand as a substitute.
Tripod
If you are recording your rehearsal with a camera or smartphone, we suggest using a TRIPOD to stabilize your recording.
Slide
Show
If you will be incorporating a SLIDE SHOW into your presentation, try to record your rehearsal using that same slide show. For most people, the slide show will be projected onto a screen beside or behind them. Do your best to set up your slide show projector so that your camera will record both you and the screen.
Seek an Assistant
Ideally, ASK SOMEONE TO RECORD YOU. This is especially practical if you will be moving around rather than standing in one spot.
Record Yourself
If you cannot find an assistant, RECORD YOURSELF. Set up the camera, set your recording on a timer, and then move back to your standing position to begin your presentation.
Proper Distance
Ensure the recording device is close enough so that your VOICE IS RECORDED CLEARLY, yet far enough away to visually CAPTURE YOUR FULL BODY MOVEMENTS and your SLIDE SHOW (if you are using one).
For additional tips on presenting effectively to an audience, we suggest you review our GUIDE and ACCELERATOR KIT which can be purchased on our website.

If you are recording your rehearsal with a camera or smartphone, we suggest using a TRIPOD to stabilize your recording. A small phone tripod works well if your phone is positioned on a desk.
Slide
Show
If you will be incorporating a SLIDE SHOW into your presentation, try to record your rehearsal using that same slide show. If your slide show is projected onto a screen behind you or beside you, set up the camera so that you capture both yourself and the projected image. If your slideshow is presented on a computer screen that is shared with others, it may be difficult to record a rehearsal video that captures both you and the slide show. In that case, for the purposes of this coaching video, consider using a projector that will project your computer screen behind or beside you as your record your rehearsal.
Proper Distance
When recording with a camera at a distance, ensure the camera is close enough so that your VOICE IS RECORDED CLEARLY, yet far enough away to visually CAPTURE YOUR UPPER BODY AND YOUR HAND GESTURES. If you are projecting a slideshow onto a screen, ensure that the camera also captures the screen.
When presenting to a camera or phone that is close to you, try to position the device so that the LENS IS LEVEL WITH YOUR EYES.
Framing
Adjust the camera or phone to FRAME YOUR HEAD AND SHOULDERS and leave enough space at the sides of the frame to CAPTURE YOUR HAND GESTURES.

Tripod
If you are recording your rehearsal with a smartphone, we suggest using a TRIPOD to stabilize your recording. A small phone tripod works well on a desk.
When presenting directly to a webcam or phone, try to position the device so that the LENS IS LEVEL WITH YOUR EYES.
Slide
Show
If you will be incorporating a SLIDE SHOW into your presentation, try to record your rehearsal using that same slide show. If your slide show is projected onto a physical screen behind you or beside you, set up the camera so that you capture both yourself and the projected image. If your slideshow is presented on a computer screen that is shared with others, record your screen rehearsal and include your slide show along with any other screen shares that will be part of your actual presentation.
Framing
If for some reason you will be presenting a slide show or chart that is physically behind or beside you, position yourself further back from the camera so that you are FRAMED IN A WIDER VIEW. If you do not need to refer to items behind or beside you, adjust the camera to frame YOUR HEAD AND SHOULDERS. For closer shots, be aware to always try to KEEP SOME SPACE AROUND YOUR HEAD IN THE CAMERA FRAME.
When using a smartphone to record your rehearsal, try to turn your phone 90 degrees sideways to record in ‘LANDSCAPE VIEW’ ( ).
If you record vertically in ‘PORTRAIT” VIEW’ (), then you will limit the screen space in the recording.

Slide
Show
If you will be incorporating a SLIDE SHOW, CHART, PROPS or OTHER ITEMS into your presentation (either as a screen share or displayed physically beside or behind you), try to record your rehearsal using the same slide show and items.
Proper Distance
If you are recording at a longer distance, ensure the recording device is close enough so that your VOICE IS RECORDED CLEARLY, yet far enough away to visually CAPTURE YOUR FULL BODY MOVEMENTS or any references to a nearby slideshow, chart, props or other items.
Framing
Adjust the camera to FRAME YOUR FULL BODY (good for showing full gestures, or for referring to a slideshow or items near you), YOUR UPPER BODY (good for gestures above the waist), or your HEAD AND SHOULDERS (good for creating a more personable atmosphere). For closer shots, always try to KEEP SOME SPACE AROUND YOUR HEAD IN THE CAMERA FRAME.
